LACH offers a Nursing Home Transitional Program (NHTP) to help individuals who need a nursing-home level of care remain safely in their homes or transition back from institutional settings. The program offers a range of services including personal care aides, home health aides, environmental modifications, assistive technology, and support with daily living activities. 

We provide specialized, comprehensive pediatric care at home for children with medical needs, allowing them to remain in a safe, familiar environment while still receiving the professional support they need. From skilled nursing and therapy services to assistance with daily activities, our goal is to support both the child and their family-- promoting comfort, development, and quality of life at home.

Our compassionate Home Health Aides (HHAs) provide care to help your loved ones stay healthy and well-cared-for. Each aide is closely supervised by a Registered Nurse, ensuring your loved ones can maintain their usual activities and lifestyle at home without compromise. LACH’s HHAs are thoroughly vetted and trained to ensure they provide your family the highest level of care.

We provide compassionate, comprehensive geriatric care at home for older adults, helping them remain in the comfort and security of a familiar environment while receiving the professional support they need. Our team offers skilled nursing, medication management, and assistance with daily activities. We work closely with families to promote safety, independence, and quality of life.

Our aides and assistants can also assist with personal needs like homemaking, housekeeping and other related services. They can help with meal preparation, feeding, shopping, light housekeeping (e.g. laundry), and medication reminding. This support helps ease some of the daily burdens on families, allowing them to focus more on their loved one’s general well-being.

LACH provides skilled home nursing visits for our clients. These licensed nurses support complex medical needs such as medication administration, wound care, feeding tube management, and vital sign monitoring. These routine visits help ensure continuity of care, reduce hospital visits, and give families peace of mind by adminstering consistent professional support right at home.
